Immunizations

All children attending school must be in compliance with Indiana Code 20-24-4-2. School districts are mandated to enforce this. Students not meeting requirements for immunizations will be excluded from school until the required documentation is provided. (IN Code 20-34-4-5) It is the responsibility of the parent/guardian to provide the immunization records to the school.

Reminder: All students entering 12th grade are now required to have two Meningococcal Vaccines and two Hepatitis A Vaccine. All students in grades K-12 are now required to have two Hepatitis A Vaccine (New 2020-2021) . All vaccines should be completed prior to the start of the school year. If there are any questions regarding immunizations, please contact your school's nurse.

Chicken Pox: For students K-5, a signed statement by a healthcare provider documenting the diagnosis of varicella and date is required. For students 6-12, parents may send in a note noting the date of the disease and parent/guardian signature.

Religious and medical exemptions must be in writing and submitted yearly. The information included on these forms is required by the Indiana State Department of Education. Please print both forms and return to the school. (Forms are below)
