Vision and hearing screenings are required by the Indiana Department of Education. Screenings listed below will be completed unless there is written notice from a parent/guardian requesting a student be excluded.

All students in grades 1, 3, 5, & 8.
A modified clinical technique is performed for first grade by volunteer optometrist (if available) |

|
All students in grades 1,4, 7, & 10. |

| Dental |
|
As scheduled with dental office
(usually grades 2, 5, & 7) |

If deviation from the normal limits is found, the parent/guardian will be notified in writing, by email, or by phone so that follow-up care with a physician/health care provider may be done.
